Hypotension can be defined as systolic blood pressure less than 90mmHg in adults. Shock is indicated by symptoms of inadequate perfusion: pale, cool, and clammy skin, change in mental status, ischemic chest pain, etc.

EMR - Emergency Medical Responder
  1. Evaluate ABC’s
  2. Apply oxygen or support ventilations.  Monitor airway and breathing closely.
  3. Place the patient in Trendelenburg position as tolerated by airway control and respiratory effort.
  4. Call for ALS intercept
EMT - Emergency Medical Technician  Perform/Confirm All Above Interventions
  1. Initiate early transport with ongoing assessment
  2. Assess blood glucose for any altered mental status.  Treat appropriately
  3. Call for ALS intercept and notify Medical Control
AEMT – Advanced Emergency Medicine Tech   Perform/Confirm All Above Interventions
  1. Cardiac monitoring, consider 12-lead ECG to identify rhythm if trained to do so.
  2. Establish IV access and contact Medical Control for fluid orders
  3. Initiate early transport
  4. Call for ALS intercept
Intermediate - Perform/Confirm All Above Interventions
  1. Initiate transport
  2. Establish IV access en route to hospital.
  3. If systolic BP < 90mm Hg or, patient’s skin is cool, diaphoretic and clammy, with clear lungs, administer a fluid bolus of 500 mL.
  4. Contact Medical Control for further fluid orders
